
Acute limb ischaemia (called ALI for short) is a common emergency condition where the arteries supplying blood to the legs suddenly become blocked due to an embolus or thrombus. Without blood to the legs the nerves, muscles, and skin die within hours, leading to amputation or death. One in ten people treated for ALI in the NHS will die in hospital within a few days. Four in ten will die within one year. ALI is one of the most common causes of amputation in the NHS.
ESTABLISH is a multicentre randomised controlled trial, funded by the NIHR HTA comparing open surgery with endovascular (minimally invasive) surgery in 470 participants with acute limb ischaemia (ALI).
The primary (clinical) outcome is amputation free survival 6 months after surgery.
ESTABLISH led by Mr Sandip Nandhra based at Newcastle University and will start in October 2025 with recruitment of participants expected early 2026.
